dates on plaque do not agree:

English of June 23, 1974 corresponds to 3 Tammuz 5734
Hebrew has 24 Sivan 5733 corresponds to June 23, 1973

I could not locate a grave for this person, so I assume that the English info is accurate and the Hebrew is mistaken.
