English: Sep 10, 1961, should be Elul 29 5721

Hebrew: Elul 29 5722, should be Sep 28, 1962

I'm assuming that the English date is the correct one and that no one paid attention to the typo in the Hebrew date. Thus I'm correcting the Hebrew date to 5721 to match the English date.
