The plaque has a different father's name and slightly different spelling of the deceased name.

His father's plaque is just below his -- Harry Nagel is clearly called Chaim in his plaque (and his grave at Sharon Memorial Park).
Conrad's hebrew name was probably Akiva, but that is only a guess. It could also have been something like Yaakov.
Finally, Conrad's grave has the symbol for Kohen (the hands in the traditional pattern) yet neither his plaque nor his grave mentions the well known phrase "HaKohen".
The assumption is that in 2002 when Conrad Nagel died, the surviving relatives had lost knowledge of Hebrew names of the family.
