Hebrew date has a typo. Dec 17, 2002 corresponds to 12 Tevet 5763. The plaque has 17 Tevet 5763. 

English date is confirmed on the grave https://www.findagrave.com/memorial/122654537/herman-gilman (Sharon Memorial park). Based on this we corrected the Hebrew date to fix the typo. 

Another instance of vendor issue with quotes. The plaque shows "Hank" but the database does not accept quotes
